Service Description

 MachForm is a web-based application that makes it easy to create and manage forms of all types: surveys, information requests, applications, reference requests, and more.  Many campus departments use MachForm to migrate paper-based forms to online forms that can be easily accessed and efficiently processed.

Available to

Faculty, staff, students at Log in using your NetID and password.


This service provides the following:

  • Training
  • Troubleshooting
  • Testing
  • Process review of how you intend to use a form
  • Assistance creating a form
  • Feedback and Suggestions regarding your form’s design.
