Service Description
MachForm is a web-based application that makes it easy to create and manage forms of all types: surveys, information requests, applications, reference requests, and more. Many campus departments use MachForm to migrate paper-based forms to online forms that can be easily accessed and efficiently processed.
Available to
Faculty, staff, students at https://forms.brockport.edu. Log in using your NetID and password.
