Service Description

This form is used to report all potential electronic information (computer) security incidents including but not limited to, misuse by authorized users, unauthorized access, computer viruses, worms, hacking, information system failure, and theft. 

Our Service Desk staff will receive your report and either assist you directly or make sure your request reaches the appropriate team.  For any information security incident requiring an immediate response please contact the IT Service Desk by phone at 585-395-5151 Option 1.

All incidents involving threats to personal safety or theft should be reported to campus police.
